A Program for Seniors and Adults with Disabilities
At Pathways Adult Day Center, we are dedicated to providing a safe, engaging, and nurturing environment for adults who need assistance with daily living, medical care, and social interaction. Our program is designed to promote independence, enhance well-being, and prevent the need for institutional care.

Service Offerings
✔ Medical Support Services- help manage medications and other daily clinical needs ✔ Therapeutic Activities-exercise and mental interaction for all participants. ✔ Personal Care-helping with toileting, grooming,eating,and personal activities of daily living (ADL) ✔ Social Activities interaction with other participants in planned activities appropriate for their conditions. ✔ Nutrition-offering Breakfast, lunch and snacks; therapeutic diets provided when prescribed by physician. ✔ Transportation- assists in identifying and arranging transportation services for clients ✔ Field Trips


Community Based
Adult day center program offering a variety of health, therapeutic, and social services to handicap, disabled and seniors who need assistance or require supervised care in a protective setting during daytime hours. ✔ Restore or maintain optimal capacity for self-care to frail elderly persons or adults with disabilities. ✔ Delay or prevent inappropriate or personally undesirable institutionalization.​

Program Description
The Program stresses partnership with participant, the family, the physician, and the community in working towards maintaining personal independence. In some situations, individuals already institutionalized may be placed back in the community with ADHC assistance and support services. Pathways ADHC has a multidisciplinary team of health professionals who conduct assessments of each potential participant to determine and plan the ADHC services needed to meet the individual's specific health and social needs.
